ICSI Procedure: Step-by-Step

The ICSI procedure is performed as part of an IVF cycle and involves several carefully planned stages. At Kiran Infertility Center, each stage is monitored by the fertility and embryology team according to the patient's individual treatment plan.

1. Fertility Assessment

The process begins with a detailed evaluation of both partners. This may include hormone tests, ultrasound, semen analysis and a review of previous pregnancies or fertility treatments. The results help determine whether ICSI is appropriate and guide the treatment plan.

2. Ovarian Stimulation

The female partner may be prescribed fertility medications to stimulate the ovaries and develop multiple mature eggs. Regular ultrasound scans and blood tests are used to monitor follicular growth and adjust medication when required.

3. Egg Retrieval

When the eggs are ready for collection, a trigger medication is given. Egg retrieval is then performed using an ultrasound-guided procedure under appropriate sedation. The retrieved eggs are assessed in the embryology laboratory.

4. Sperm Collection or Surgical Retrieval

A semen sample is collected and prepared for treatment. In cases where sperm is not present in the ejaculate or cannot be collected adequately, techniques such as TESA, TESE, micro-TESE or PESA may be considered based on the underlying condition.

5. Sperm Preparation & Selection

The sperm sample is processed in the andrology laboratory. Suitable sperm are selected based on factors such as movement and appearance before being prepared for ICSI. Additional sperm-selection techniques may be considered in selected cases.

6. ICSI Microinjection

During ICSI, an embryologist uses specialised micromanipulation equipment to inject a single selected sperm directly into a mature egg. This direct injection is intended to assist fertilisation when conventional IVF may be less suitable.

7. Fertilisation & Embryo Development

The eggs are assessed for fertilisation after ICSI. Fertilised eggs are then cultured in the laboratory and monitored as they develop into embryos. Where available and appropriate, embryo development may be monitored using time-lapse technology.

8. Embryo Transfer

A suitable embryo may be transferred into the uterus using a thin, flexible catheter. Depending on the treatment plan, suitable embryos may also be frozen for future use.

9. Pregnancy Test & Follow-Up

A blood pregnancy test is performed after the recommended interval following embryo transfer. The fertility team provides follow-up guidance and support while monitoring the patient's progress.
